National Water Supply Survey
The Environmental Protection Agency, along with the Department of Natural Resources (DNR) has mandated an inventory of all water service lines. Please complete the form below. Any service line that does not have a completed form will require an onsite inspection by City of Alton staff.
This survey is divided into several sections and should take about 5-10 minutes to complete, if you are able to inspect the areas of pipe required to complete this survey.
If you are unsure about pipe types, or where to look for answers, the survey will guide you through that in Step 3.
